    Fixed Benefit Plan

    The Fixed Benefit Plan through Aetna is available to newly benefits-eligible employees who are subject to the 90-day waiting period for state-sponsored health insurance.  Elections may be made for employee, employee plus one or family.

    Unlike major medical insurance, the Fixed Benefit Plan is an indemnity plan which pays a fixed amount per healthcare service.  The Plan has limits on the type and frequency of services covered (including prescriptions) and is designed to offset a small portion of incurred healthcare costs.  Please carefully review the Summary of Benefits.

    Premiums are 100% employer paid.  The employer contribution is considered imputed income and carries a nominal increase to taxable gross income.  Click here for more information on imputed income. 

    Coverage under the Fixed Benefit Plan terminates automatically at the end of the 90-day waiting period for health insurance.  Click here for more information on health insurance options through the Arizona Department of Administration.

    TO ENROLL:
    Call Human Resources at 520-621-3662, option 3 and indicate that you would like to enroll in the Fixed Benefit Plan.  An online enrollment event will be opened for you in UAccess Employee.  Then you may log in to UAccess Employee and select Self Service > Benefits > Benefits Enrollment.  Enrollment must be completed within 31 days of your hire date.  Coverage will be effective the first day of the pay period following your enrollment.

    If you are enrolling a domestic partner or the child of a domestic partner, you must complete additional affidavit and tax status forms.

    NOTE:  After enrolling in the Fixed Benefit Plan, you must also complete an online enrollment for the health insurance plans that will be effective at the conclusion of the 90-day waiting period.
