    FSA Dates and Deadlines

    • This plan works on the calendar year (January 1st, or new hire effective date, through December 31st).
    • Newly hired/newly eligible employees must enroll in this plan within 30 days of hire date (or benefit-eligibility date), or during the annual open enrollment period for the upcoming calendar year.
    • Annual Open Enrollment is generally held in November each year. Open enrollment elections take effect January 1 of the following year.
    • Claims incurred during the 2011 plan year must be filed by April 30, 2012. Any expenses incurred in 2011 that are not claimed by April 30, 2012 will be forfeited.
    • ALL employees in the plan MUST re-enroll each year in order to continue participation in the new calendar year. (All enrolled members will receive re-enrollment information from ASI, the plan administrator, each year by mail during the annual open enrollment period for this plan.)
