    Group Long Term Care Insurance - Enrollment for University of Arizona Retirees

    Retirees must complete a paper application. Application forms may be ordered by calling Prudential toll free at (800) 732-0416 and requesting a University of Arizona enrollment kit. The application forms are also available online using the Prudential Group Long Term Care Insurance website. The Group Name is university and the Access Code is arizona. (Both are case-sensitive.)

    Note: You must use a Windows-based browser, such as Internet Explorer, to complete the Prudential online application. Windows-based computers are available at Human Resources in USB Room 114, University Main Library, Arizona Health Sciences Center's Medical Library, and the Office of Student Computing Resources.
    (NOTE: Both Group Name and Access Code are case-sensitive)
