    Medical Insurance

    University of Arizona employees, including those with same sex domestic partners, have several medical insurance options available to them through the Arizona Department of Administration (ADOA) Benefit Options.  

    Employees who would like to enroll an opposite sex domestic partner as a dependent are eligible for medical insurance through the University of Arizona's Alternative Plan.


    ADOA BENEFIT OPTIONS PLANS
    Employees (including those with same sex domestic partners) may choose:

    EPO Plan - Aetna, Blue Cross Blue Shield/Ameriben, Cigna or UnitedHealthcare
    PPO Plan - Aetna, Blue Cross Blue Shield/Ameriben, or UnitedHealthcare
    HSAO Plan - Aetna

    If you elect any Benefit Options medical plan, MedImpact will be the network you use for pharmacy benefits.  Enrollment is automatic.

    It is important to review the Benefits Enrollment Guide for more detailed information, including plan comparisons, prior to making your selections.

    If you are enrolling a same sex domestic partner, you must complete additional forms.
    If you are enrolling a child of a same sex domestic partner, you must complete additional forms.
    Imputed income for non-tax-qualified dependents

    Health Insurance ID - Your health insurance ID is a 9 digit number that is used as identification for ADOA-sponsored insurance.  Your health insurance ID is different from your EmplID.  To learn your Health Insurance ID, visit UAccess Employee and select Employee/Manager Self Service > Self Service > Benefits > Benefits Summary.

    UNIVERSITY OF ARIZONA ALTERNATIVE PLAN - UNITEDHEALTHCARE HMO
    The University of Arizona Alternative Plan is only available to employees who will be enrolling an opposite sex domestic partner.

    If you elect this plan, Medco will be the network you use for pharmacy benefits.  Enrollment is automatic.
