Elect electronic delivery of your IRS tax form 1095-C


Please take a brief moment to choose electronic delivery of your IRS 1095-C form. The 1095-C helps you report health insurance coverage to the IRS when you complete your tax return, and electronic delivery is the most efficient way to receive the form. 

To elect electronic delivery, visit UAccess Employee at uaccess.arizona.edu. Once on the UA Employee Main Homepage, select the Benefits tile and then click on the 1095-C Consent option. 

Even if you already receive your W-2 form electronically, you will need to complete a separate request for electronic delivery of the 1095-C.
