Who to Contact for What

To request a new or replacement badge, visit then log-in and submit a work order. (If you need assistance with making a work order, please click the link below in step 1).

  1. Submit a work order requesting a badge. Fill in as much information as possible to assist the Campus Management and Operations Team process your request.  

    Ensure the correct location is entered and select Badge (New or Replacement) option as the problem. 

    Then, for "Short Description" please provide as much detail as you are able (consult your supervisor) about your location and your access needs. 
  2. Badge photos are taken on the 1st floor of Building 1 during walk-in hours or by appointment only. To make an appointment, please call 602-827-2758
    Walk-in Hours: Tuesday and Wednesday 9:00 AM - 12:00 PM and Thursday 12:00 PM - 3:00 PM

Please fill out the form below if you have a project that may need assistance. The request will be reviewed by the Project Support Team to determine eligibility for inter-departmental support. Once evaluated, projects will be assigned and completed based on scope, time-sensitivity, and capacity.
