Background Check Secure Document Upload

Use this secure document upload below to electronically deliver documents to the Pre-Employment/Background Check Team.

Instructions:

• Please ensure your documents are carefully and thoroughly completed before uploading your document files.
• When saving document files, please label your file in the following format: Last Name First Name – Document type.  For example:
  • Smith John – Consent form
  • Smith John – Disclosure form
• You will see a “success” message to confirm the documentation is uploaded.  Due to the volume of uploads received, please do not contact our office to validate receipt.
• Do not fax, mail or email documents that are uploaded to this location as this results in duplicate efforts.
