Dental & Vision Plans
The Arizona Department of Administration provides medical, dental, vision, and disability insurance to state employees, including University of Arizona employees and their legal dependents.
Domestic Partner Plans
The State of Arizona does not offer benefits to the domestic partners of employees. However, the University of Arizona offers medical, dental, and vision insurance, dependent life insurance, and Domestic Partner Tuition Remission (at the University of Arizona only) to these families.
Health Reimbursement Arrangement for Fertility Treatment
Amount
Eligible employees may receive up to $2,500 through the University of Arizona Health Reimbursement Arrangement (HRA) for certain fertility treatment expenses that are not covered by the State of Arizona health plans. This is a one-time household benefit.
Eligibility
HRA funds may be claimed for services provided to employees or dependents enrolled in the Arizona Department of Administration High-Deductible Health Plan or PPO Plan. Employees enrolled in the High-Deductible Health Plan must first meet their annual deductible before becoming eligible for HRA funds.
Reimbursement
Fertility treatment services must first be submitted to your health insurance carrier. If the claim is denied, contact hrsolutions@arizona.edu for instructions on how to enroll in the HRA and submit a reimbursement request.
