Please complete the following form for employment verification.

Per policy, Employee Records can only provide the most recent employment information, which includes Name, Title, Dates of Employment, Employment Status, Department, Full-time/Part-time and Salary. We do not provide detailed title history. We do not require a signed release for this information.

If you require verification of job history or income history, please submit your request to our Payroll Office using their secure upload feature at https://financialservices.arizona.edu/payments/upload. This process allows a secure way to submit income/salary forms.
