For current or past University of Arizona employees, only select “University of Arizona” option below. For current or past UA Global Campus (UAGC) employees, please select all options below that apply.

You must submit a separate form for each employer to your PSLF servicer. If you select multiple employers, you will receive separate Adobe Sign agreements to complete for each employer.

For example, if you were hired at UAGC in 2021 and have been continuously employed by UAGC since then, select "UA Global Campus (UAGC)" and "University of Arizona." Or, if you were only hired with UAGC on 7/1/2023 or after, then you would only select "University of Arizona".

If you were employed by an organization prior to UAGC and are interested in completing a Public Service Loan Forgiveness (PSLF) form, please read the information below.

The University of Arizona Global Campus (UAGC) became an entity in December 2020. Employment prior to that we are unable to validate in Section 5A of your PSLF request.

You may still submit a PSLF form to the government by following the steps below.
