State Dental and Vision Plans

The Arizona Department of Administration provides medical, dental, vision, and disability insurance to state employees, including University of Arizona employees and their legal dependents. This page outlines just the dental and vision options. For medical options, visit the State Medical Plans webpage.

  Note: The following plans are not available to employees who wish to enroll a domestic partner. Please visit the University Alternative Plans webpage for more information about domestic partner coverage.



Dental Insurance

You have two dental insurance options. Download the comparison document, or review the information below.

Dental Plan Comparison (PDF)

UnitedHealthcare Solstice - HMO

  • You must choose an in-network primary dental provider.

  • No provider networks in these states: AK, AL, AR, DE, HI, IA, ID, LA, ME, MS, MT, ND, NE, NH, OK, RI, SD, VT, WV, and WY, nor in these territories: GU, PR, and USVI.

  • Reduced costs for services (fixed co-payments).

  • There is no annual maximum benefit for the plan year.

Delta Dental - PPO

  • May see any licensed dentist (cost are lower for in-network dentists).

  • You pay a percentage of the cost for services.

  • Preventive services are not subject to the annual deductible.

  • Restorative services are subject to the annual deductible, then plan pays a percentage of post-deductible expenses for basic and major restorative services.

  • The annual maximum benefit for a plan year is $2,000 per person.

Vision Insurance

You have the option of selecting the Avesis vision plan.

Visit the Avesis website.

  • Participating network providers are contracted with Avesis to provide services at a reduced rate for Avesis members. You may choose from a participating network provider or an out-of-network provider. Out-of-network providers may result in higher out-of-pocket costs.

  • Co-payments, deductibles, or coinsurance fees may be charged when you use services.

  • When using out-of-network providers, you must pay the provider and submit an itemized statement to Avesis for reimbursement of covered expenses.
