Protect Yourself; Get a Flu Shot

Flu season is approaching. By getting your seasonal flu vaccine, you can help protect yourself, your family, and your community from the flu.

Steps To Get a Flu Shot

  1. Locate your Alternate Employee ID in the University Benefits tile of UAccess at the top of the Benefits Summary tab. Your Alternative Employee ID confirms that you are eligible for benefits as a State of Arizona employee. Please note that your Alternate Employee ID is NOT the same as your Employee ID, even though they may look similar.
  2. Download the Flu Shot Consent Form (PDF); Add your Alternate Employee ID number to the Flu Shot Consent Form in the appropriate box under "State Employee Information".   
  3. Get your flu shot at the location most convenient to you. On- and off-campus clinics and options are available. Visit the Life & Work Connections Flu Shot Clinics webpage for options and instructions.
