New Leave of Absence Request Process Streamlines Leave Requests

As of July 1, Human Resources simplified the process for requesting a leave of absence for personal health or family reasons. 

Employees will now use a single, confidential Leave of Absence Request Form to submit their request for Family and Medical Leave (FML), Parental Leave, Military Training Leave, Extended Military Leave, Personal Leave (Staff), and Extended Personal Leave (Appointed) directly to a leave specialist. This change ensures that leaves for personal health or family reasons are in compliance with federal and state guidelines and University Policies. 

Supervisors will continue to approve vacation, sick time, bereavement, jury duty, and volunteer service. The Office of the Provost will continue to handle approvals for sabbatical and prestigious awards leave for faculty.

Once you submit a leave of absence request form, it will be routed to a leave specialist who will assess your eligibility, provide the necessary forms, and guide you through the next steps, if needed.
